Our goal at Herzig Eye Institute for every patient having cataract surgery is to achieve their best possible vision. Most of our patients want to eliminate or significantly reduce their dependence on glasses after their cataract surgery. Some patients are happy to continue wearing glasses and can proceed with traditional cataract surgery, which is covered by OHIP in the province of Ontario.
In order to achieve independence from glasses, cataract surgery is combined with refractive surgery. Refractive surgery is defined as surgery to eliminate or reduce dependence on glasses. When refractive surgery is combined with cataract surgery it consists of the following:
All the above elements of refractive cataract surgery are essential to achieving independence from glasses. If a patient chooses to eliminate or reduce their dependence on reading glasses after cataract surgery with the aid of a multifocal IOL or Monofocal+ IOL, then it is necessary to use all these refractive aids to achieve the desired result.
The most recent exciting advance in IOLs is the introduction and Health Canada approved Light Adjustable Lens (LAL). The LAL allows the surgeon to change the power of the lens 3-4 weeks after the surgery by applying UV light to the lens with precise technology for a few seconds. This adds another level of accuracy to the final visual result.

Herzig Eye Institute specializes in treating vision problems related to aging. Over time, the eye’s natural lens can become too stiff to focus up close. As a result, words and other nearby objects appear blurry. The clinical term for this loss of near vision is presbyopia.
There are a number of options to correct presbyopia (loss of reading vision) including:
SMILE®, also known a Small Incision Lenticule Extraction, is considered the most technologically advanced procedure available while also being less invasive as previous generations of Laser Vision Correction procedures. This procedure is often used to treat patients with Myopia (nearsighted) and have astigmatism. SMILE® eye surgery is a flapless procedure that has all the combined advantages of surface ablation (PRK) and LASIK. It is PRK without pain and LASIK without a flap. SMILE is a flapless, bladeless, noiseless, odorless procedure.
LASIK stands for Laser-Assisted In Situ Keratomileusis. A wide range of myopia, hyperopia, and astigmatism can be corrected by the LASIK procedure. It is a delicate surgical procedure that allows for a very predictable healing response. Your vision stabilizes rapidly, and you can expect to have little or no discomfort following LASIK. PRK (photorefractive keratectomy) is a type of refractive procedure performed to correct nearsightedness, farsightedness and astigmatism – just like LASIK vision correction does. The LASIK surgeon use an excimer laser to reshape the cornea to allow light to properly focus on your retina which is the back of your eye. PRK is often recommended for patients who have thin corneas. Rather than creating a flap on the corneal surface (as in LASIK), your doctor removes the outermost layer of corneal tissue so the reshaping process can be administered.
The Visian ICL™ is an implantable lens made of a soft biocompatible material, similar to that of a soft contact lens. Unlike a contact lens, the ICL is permanently positioned inside your eye behind the iris (the colored part of your eye) and in front of the natural lens. In this position, the ICL remains invisible and corrects your vision without causing many of the optical aberrations that occur from laser treatments to the cornea for higher degrees of nearsightedness.
Advancements in cataract surgery have led to new technology with lens replacement surgery designed to correct refractive error. As a result (RLE) Refractive Lens Exchange is very similar to cataract surgery but has multiple benefits that extend beyond. This is a surgical procedure intended to reduce or eliminate the need for glasses or contacts for people over the age of 45, who may be experiencing new visual challenges. Refractive Lens Exchange can be used to correct presbyopia, nearsightedness or farsightedness. Since the process is very similar to cataract surgery you will never develop a cataract if the natural lens is removed during this eye surgery.
